2)  Download and fill out the appropriate coversheet and designation form for your Sector below.

3)  FAX the COVERSHEET, DESIGNATION FORM and entire DISCIPLINE PACKET (or NOTICE TO APPEAR form) to our office at (619) 232-1235, (760) 370-5559 or (928) 783-5843.

4)  Confirm with our office that your fax was successfully received.

2)  FAX or EMAIL the COVERSHEET, DESIGNATION FORM and entire DISCIPLINE PACKET to the NBPC National Vice-President responsible for your Region.

3)  Confirm with the NBPC National Vice-President responsible for your Region that your fax or email message and attachments were successfully received.
