If you have received a proposal for discipline or a notice to appear for an investigative interview:

1)  Call our office at (619) 232-8142 or (520) 628-8142 IMMEDIATELY.

2)  Download and fill out the appropriate coversheet and designation form for your Sector below.

3)  FAX the COVERSHEET, DESIGNATION FORM and entire DISCIPLINE PACKET (or NOTICE TO APPEAR form) to our office at (619) 232-1235 or (520) 628-8143.

4)  Confirm with our office that your fax was successfully received.
